Partial Care Day Rehabilitation Center Including Bridge to Wellness (Union): Our recovery oriented partial care program was one of the first of Bridgeway’s programs and has provided services in Union County since 1970. Recently, Bridge to Wellness programming was implemented and a primary medical care exam room opened to provide access to primary health care for persons served in the region. The program offers wellness, clinical, prevocational, educational, social, and support services as a bridge between mental illness and healthy productive active living. The program provides a range of recovery- oriented services that address the holistic needs of the person in recovery. These services focus on helping individuals to connect or reconnect with valued roles, to live as independently in the community and to prevent the need for more acute psychiatric services and urgent medical services. Residential Intensive Support Team (RIST- Union, Middlesex, Mercer/Middlesex, Tri-County- Hunterdon/Somerset/Warren): Bridgeway’s Residential Intensive Support Team (RIST) is a mobile flexible team that provides services 24/7. The team provides person centered recovery and wellness services to people in recovery who are re-entering the community from a state hospital. RIST provides rent vouchers to help individuals live in independent housing in the neighborhood they choose. RIST uses a housing first approach, working with individuals to select, get, and keep the housing of their choice. Homeless Outreach Services (Union): Bridgeway provides outreach, support, and case management services to adult Union County residents who are homeless, or are at risk of becoming homeless. Bridgeway homeless outreach services accepts referrals from anywhere in the community- individuals who are homeless on the streets, in shelters, as well as various social service agencies. Supportive Housing (Hunterdon, Union): Bridgeway Supportive housing provides outreach and support to assist individuals with living independently in their own apartment. Services include assistance with finding housing and the moving in process as well as rehabilitation services including working on developing and reaching goals. Enhanced Supportive Housing (ESH-Union): Bridgeway Enhanced Supportive Housing provides intensive support to individuals who are at risk of long term hospitalization and have a history of homelessness. ESH assists the individuals with obtaining a rental assistance voucher and with choosing an apartment in the community. Enhanced Supportive Housing provides services seven days a week. Medically Enhanced Supportive Housing (MESH- Mercer): Bridgeway Medically Enhanced Supportive Housing provides integrated primary medical care along with intensive support to individuals who are at risk of long- term hospitalization and have a history of homelessness. ESH assists individuals with obtaining a rental assistance voucher and with choosing an apartment in the community. Enhanced Supportive Housing provides services seven days a week. PACT (9 Teams: Bergen, Eastern Union, Western Union, Central Union, Hudson, Hunterdon/Warren, Passaic Combined, Passaic, Somerset): The Program for Assertive Community Treatment (PACT) is a mobile psychiatric rehabilitation program designed to help people with psychiatric disabilities live in the community. PACT is a 24- hour a day, 7-days a week service designed to respond to the individual needs of each person served.
