Bridgeway Rehabilitation 50th

Justice Involved Services (Union): Bridgeway Justice Involved Services (JIS) operates on the belief that a comprehensive diversion program can make a difference in breaking the cycle of arrest, re-arrest and incarceration of individuals with serious mental illnesses charged with non- violent offenses. Services are a blend between Homeless Outreach and Supportive Housing. Career Services (Sussex, Union): Bridgeway emphasizes career development as a crucial ingredient in the rehabilitation of people who have mental illness. Services include Supported Education and Supported Employment, an evidence based practice. People are assisted using a “choose- get-keep” model beginning with a readiness assessment. Services are offered “where and when needed” – at school, on the job, or wherever the person served chooses. Community Support Team Including Bridge to Wellness (Sussex) : The Community Support Team (CST) is comprised of 5 program elements (Partial Care, Supported Employment, Integrated Case Management (ICMS), Supportive Housing and Homeless Outreach). CST serves adults in Sussex County who experience mental illness that has interfered in some major way with carrying out their choices in living, learning, working or socializing in their community. The ICMS program primarily serves persons who have been discharged from state psychiatric hospitals. ICMS is designed to link people to services including mental health, social services, entitlements and medical care. For Sussex Supportive Housing, Supported Employment and Partial Care programs please see above for program descriptions. PESS – Psychiatric Emergency Screening Services (Somerset): Also known as “Mobile Screening.” PESS is Bridgeway’s newest addition to programs offered in central and northern NJ. PESS offers rapid mobile outreach and hospital diversion, a community based psychiatric emergency screening service center that will include a “Living Room” model of care for services, planning, and follow-up appointments. PESS also offers use of flexible follow-up PES services, on-site and in-vivo. Crisis Intervention Services (Hudson): The Early Intervention Support Services, or Crisis Intervention Services (CIS) program acts as an Urgent Care Center for individuals experiencing a mental health crisis or a worsening of a mental health disorder who are at risk of utilizing emergency services. CIS assists individuals in remaining in the community by providing rapid access to short term, recovery-oriented crisis stabilization and intervention services. This 24- hour, 7 day a week service is designed to respond to the individual needs of each person served. For some, one or two visits are sufficient, while for others the maximum enrollment period of 30 days is beneficial. I’ve spent my whole life being depressed and with mental illness… For the first time in my life I have a whole team that has supported me and unconditionally accepted me. Bridgeway saved me.
