The PACT model was founded in Madison, Wisconsin in the 1970s. Bridgeway was one of the first to deliver these services in NJ and has become a model for other community healthcare programs. Many of those treated by PACT have had involuntary mental health hospitalizations, but resist mental-health counseling in a therapist’s office setting. Many have both mental and physical health issues. The PACT teams treat each of the persons served holistically. By working together, the teams address issues ranging from wellness to employment to mental health. The team’s goal is to give people the support they need to prevent the need for hospitalization and become as productive as they are capable of being.
